Ask any counsellor who "BetterHelp" are and they will likely know who you are talking about. This US-based online therapy platform has brought therapy to a mass market, with their advertising abundant almost everywhere you look. But what is the platform really like to work on as a counsellor? Many of us may have used BetterHelp to begin our private practice or to supplement our income from elsewhere. We probably saw great incentives such as the ability to set our own working hours and caseloads, and therapist pay based on a sliding compensation scale; the more hours we work, the higher our hourly rate of pay. Many of us might remember getting excited emails from recruiters promising $1,000 sign-up bonuses. It might have looked too good to be true. And this is the case for many in reality. We launched two surveys, one in 2023 and one in 2024, that sought to find out how therapists working on the platform were really finding things. Out of 116 therapists, only 4% were consistently getting more than 20 hours on BetterHelp per week. In fact, 38% were getting less than 5 hours. Many therapists on the platform find that client allocation can be random and there aren't enough clients to go round, but some of the main issues reported are repeatedly getting DNAs or clients who don't engage very much. This means that for many counsellors we get hit in the pocket. It's not just about money, either. Many counsellors find the platform pushy and this can affect our work-life boundaries. A lot of counsellors also struggle to know where they stand with BetterHelp on issues such as safeguarding or insurance when working with overseas clients.
